Controlled Clinical Trials Of Braindirected Therapies After Cardiac Arrest

The controlled clinical trials that focus on brain injury from global ischemia in CA started with the Brain Resuscitation Clinical Trials (BRCT) in 1986 (15) and have continued with studies by the Hypothermia After Cardiac Arrest (HACA) Study Group (8) and Australian groups (9 ) in 2002, which studied the use of moderate hypothermia. The agents used in these studies were chosen based on results of preclinical animal experimentation that showed potential benefit. Further clinical studies using similar agents in related neurologic injuries also demonstrated efficacy. All of these therapies, with the exception of induced hypothermia, provided no significant benefit to neurologic or functional outcome. However, the failed clinical trials provided critical insight into the epidemiology, pathophysiology, and clinical trial design in this injury paradigm.
