Local brain cooling would be most desirable for hypothermic neuroprotection. However, the effectiveness of surface cooling has been less than desirable. Surface cranial cooling with ice during advanced cardiac life support has been shown to be ineffective in reducing brain temperature (58). One small study demonstrated the effectiveness of a novel cooling helmet in inducing cooling following cardiac arrest (59). The investigators were able to reduce core temperature to 34°C in a median time of 180 min (0.6°C/hr) following return of spontaneous circulation in patients with cardiac arrest with pulseless electrical activity and asystole.
