That has been used for years to manufacture estrogen, progesterone, and testosterone. Because DHEA is a precursor of testosterone, sometimes high dosages of it cause male characteristic side effects such as facial hair and acne, although doses around 50 mg or less do not appear to cause these side effects. DHEA has been touted as an anti-aging miracle drug. Its claimed effects include increased energy and better sex drive. Although DHEA is available over the counter, you must discuss its pros and cons with your clinician before trying it. The dosage of DHEA being used in clinical trials is about 150 mg to 200 mg per day. A clinical trial looking at DHEA's effect on Crohn's disease is also being conducted.
HEPA respirators are required for all health care workers, including phlebotomists, who enter rooms of patients with tuberculosis, and also to clean up spills of pathogenic microorganisms (see Chapter 64). All respirators should be fit-tested for each individual so that each person is assured that his or hers is working properly. Males must shave their facial hair to achieve a tight fit. Respirators are assessed for safety by the National Institute for Occupational Safety and Health (NIOSH), a branch of CDC. N95 or P100 disposable masks (available from 3M, St. Paul, Minn) are commonly used in the clinical laboratory.
Self-reported severity of facial hair and time spent on hair removal in a randomized controlled trial carried out in 88 women with PCOS (9). A retrospective study assessed 242 patients with hirsutism who received diode laser treatments over 4 years (10). After an average of two treatments (range 16), a sufficient reduction in terminal hairs was achieved for a mean period of 8 months, and the hair-plucking interval was raised from a mean of 3.7 days before treatment to 15.2 days after laser epilation. The procedure was well accepted by about 80 of the subjects. At least in the short term, ruby, alexandrite, and diode lasers or the intense pulsed light resulted in similar success rates. The long-pulsed diode and Nd YAG wavelength-based laser systems are best suited to treat patients with darker skin.
Eflornithine is an irreversible inhibitor of ornithine decarboxylase. This enzyme catalyzes the conversion of ornithine to polyamines, which are involved in the regulation of cell growth and differentiation in several tissues. The enzyme is modulated by androgens and takes part in the physiology of hair growth, regulating the proliferation of matrix cells in the hair follicle. Studies have indicated that blockade of this enzyme activity in hair follicles slows hair growth, and the drug has recently been licensed for topical treatment of facial hirsutism. Percutaneous absorption of the drug is negligible. In short-term clinical studies, eflornithine 11.5-15 cream was better than placebo in reducing hair growth in women with unwanted facial hair, as demonstrated by objective and subjective methods (11). However, hair growth returned to pretreatment rates within a few weeks after stopping treatment. Mild irritation and folliculitis may affect the skin with treatment. Anecdotal evidence...
