Complications Infection

Symptoms And Signs

Common skin warts mostly affect children and young adults, with multiple lesions on hands and fingers. Flat skin warts tend to localize to arms, knees and the face. Deep plantar warts are usually single and are seen in children and adults. Warts may occur on the vocal cords (laryngeal papillomas).

Therapy And Prophylaxis

Most skin warts regress spontaneously. Various local treatments have been attempted with uncertain effect (podophyllin, salicylic acid, glutaraldehyde, acetic acid and freezing). When the wart is troublesome, surgical removal may be necessary. Interferon has been used for treatment of laryngeal papillomas with favourable results. In approximately 50 of refractory cases, however, relapse is common. Education concerning prevention of sexually transmitted diseases should include appropriate counselling about HPV infections.

Immunopathology

Although the full understanding of the immunopathology of AD remains to be elucidated, various immune abnormalities can be routinely detected in these patients. Findings include increased serum IgE, abnormal delayed-type skin reactivity to common antigens (i.e., tetanus antigen), decreased incidence of contact dermatitis (i.e., poison ivy dermatitis) and increased susceptibility to cutaneous viral infections such as herpes simplex, verruca vulgaris, molluscum contagiosum, and vaccinia. In vitro experiments also show a decreased lymphocyte response to mitogens (i.e., phytohemagglutinin) and recall antigens (i.e., tetanus) and a defective cytotoxic T-cell response. Reduced chemotaxis of monocytes and polymorphonuclear leukocytes has also been reported in AD. These data indicate that a combination of mechanisms may be important in the immunopathogenesis of AD (Fig. 2).
